INTRODUCTION
Value-based care (VBC) is a healthcare delivery and payment model that aligns reimbursement with patient outcomes and quality of care, as opposed to the traditional fee-for-service model, which compensates providers based on the quantity of services delivered.
VBC presents an opportunity to improve health outcomes, while ensuring financial sustainability. Transitioning to VBC requires planning, infrastructure development, and ongoing performance management.

Organizing framework that offers structure and support in health center’s transition from volume-based to VBC. Change Areas can be targeted for improvement to transform systems and enhance overall performance.
